We now ignore any images that are only 1 or 2 pixels high - this was a
problem as some images (eg the toki on the cover of our manuals) would get
converted into hundreds of very thin images. This also really speeds up
the conversion for some pdfs as we don't need to call pnmtopng for all
these images.

Added -DUSE_GZIP and -DHAVE_POPEN to the default cflags. popen() is posix.
